4. Official and sealed diploma and certificate of graduation from the latest degree granting institution (official school documents with a copy of English transcription notarized)

5. Official and sealed transcript from the latest degree granting institution (official school documents with a copy of English translation notarized)

6. Two 2”x 2” pictures (digital pictures available)
